Attention Deficit Hyperactivity Disorder (ADHD) is a term used to describe and encompass multiple diagnoses, including inattentive and hyperactive subtypes. It is not a service covered by MSP and therefore, a fee is charged for the professional time it takes to review your child’s chart and complete the application.

Even when recommended by a CYSN Worker, accountant, your child may not qualify for the DTC. Please review the application in detail and determine whether your child meets criteria BEFORE asking your doctor to complete the form. It is extremely helpful for you to provide examples of impairments and report the severity and impact of the illness for the doctor to complete the form.
